Requirements to be a Volunteer

It is your responsibility to ensure that the coaches and volunteer have met the requirements below:

1. All volunteers must undergo a background check including fingerprinting. The check is conducted by an approved agency. Results of the investigation are submitted directly to the pastor and remain confidential. Anyone convicted of a felony will not be permitted to act as a volunteer in the Association. Talk to an athletic association member for the proper procedure. The athletic association will pay the fee for the background check.
2. All volunteers who work with children must take the VIRTUS training, part of the "Protecting God's Children" initiative. See the VIRTUS website for more information on the VIRTUS program. Check here the Cleveland Diocesan VIRTUS website, which includes schedules for VIRTUS training sessions.
3. Coaches must take the Coaches Development training offered by C.Y.O. The athletic association will pay the fee for coaches to take this training. It is usually offered in conjunction with VIRTUS training.
   
   C.Y.O. maintains an online database of coaches who have completed the coaches development training.

Sports Registration Form

The Sports registration form is required if not registering online.

Photo Release form

Form to obtain permission from a parent or guardian of a minor child to photograph and use photographs of children in printed or electronic formats. Examples for use include newsletters, sport-related websites, etc.
