Required Forms for C.Y.O. Sports

Medical Exam and Emergency Release

This form is required to be on file before a child may practice or play any C.Y.O. sport. It is not required for intramural sports. This form must be re-submitted annually.

Sports Registration Form

While sports registration can be easily done online, we provide a form for mailing or drop off at school. Follow the instructions on the form and fill it out completely. If you registered online you do not need this form.

Player Eligibility Form

If your child belongs to an adjoining parish but would to play on a WPCA team this form must be filled out and approved by the pastors of both parishes. Normally the sport must not be available at the player's parish/school.
